Amayumero is the song.
Except it involves Rin, Miku and Len. Not Meiko.
Or Meiko could just be another geisha, whom len "visited."
She just wasn't seen in the song.

not sure why Meiko is in it with Len...
the story is about Rin and Miku that made a promise to never leave each other as they were kids.They are later adults and they're prostitutes(can't find a kinder word right now).Rin one day comes across Miku with a man(Len) and he's in 'love' with her(i think).Miku stays with him for some reason and they decide to run away together.Rin is then upset because their promise was broken and she continues to wait for Miku to come back.
i'm pretty sure this is the storyline but i'm not completely sure beause it was never said...
I'm guessing Meiko would be another prostitute who he 'loved' too...?

I think this picture is actually from Amayumero's(Rain Dream Tower)sequel: It's Koiyamiro (Dark Love Tower, look it up, it's aweasome!). Len does mention a man with a brush(Kaito)in the song...
Also, the woman with Gackupo looks like Luka... just saying.

Okay, judging from Koiyumero's lyric I found in Kisekikui's video translation of Amayumero and this image, the person Len truly loves is Meiko, another prostitute.
Apparently she is taken, so he is waiting for her contract to end.
In Amayumero, Len's spent days with Miku is him attempting to hide or,at least, reduce his pain or something.
Such a Casanova you are, huh Len. Poor Miku died waiting for you and Rin suffered too, you know?
Anyways, as much as I love Hitoshisuku's songs, he never lets the couple get together. Huh... Kinda makes me worry for Rin and Len in Karakuri Burst and its sequels and/or prequels... They are lovers in this series.
After watching and thinking over and over again, I think that's pretty much it... >in the picture, the person waiting in the rain is Len.)
Also, the first man (Gackupo) and the secound man (Kaito) are her previous clients.

I think that Len loved Rin but her contract wasn't going to finish soon,so he tries to love miku,that understanding that len doesn't love her dies ,then he tries with meiko but it doesn't work,too. (rinxlen theory,yup)

This picture is not from Rain Dream Tower. It is from Dark Tower Love; Len's version of the song. Miku is the one in the background and that is MEIKO that Len is holding. KAITO is also shown in the background of the picture, but that is also showing Len holding Miku; as a flashback of when Len told Miku he loved her. Not Gakupo. Luka and Gakupo aren't even in this song's story. Len is with MEIKO because he left Miku for her.
It is just Rin, Len, Miku. KAITO is there partially for Dark Tower Love.
Please, look up your information before you put it there.

Gakupo in the picture holding on Luka's hands might be Len's first brother, who is very gentle and caring (2nd verse).
Kaito is the second brother, who is holding on the brushes, and is referred to be like the wind because of his freedom of doing things and thinking about the future (3rd verse).
Meiko is Len's first love, also a prostitute (oiran), who is seemed to be still bounded by contract with another client so she has to refuse Len (that might be the reason why in the last two sentences in the last verse, he talks about the waiting for her end of indenture). However, Len still loves her so much that even when he tries to find another "flower" (Miku) to comfort him in order to forget Meiko but he just cannot do it ("the wounds which will never heal" - 6th verse).
In the last verse of the song, he tells us about someone waiting for him, this can be Miku who he promised in the song 'Amayumerou'. On the other hand, it is said that his heart still belongs to Meiko and currently waits for her end of contract (as I have said above). And this could be why Len never goes back to Miku after he promised to return for her, despite Miku's waiting for him.
Therefore, the girl in the picture above is indeed Meiko, Len's first love and "fated one".
His perspective of the story kept changing in this song so it was a bit confused for me at first. I am not sure if I am right or not but I hope I helped somebody. :)

P.S: Also, for those who do not know, Suzunosuke published an illustration book which has this one picture of Len hugging Meiko (who is tied by ropes just like the picture above, implying more of her being bounded by the contract or something that Len cannot do anything about it, since his face in this picture is very sad and kind of powerless) from behind, which makes my guess for Meiko being Len's true love is true. (12th pic from left to right)
